I am immensely excited about this opportunity to join such a well-respected and innovative organization. As a Community Health Worker, I would be responsible for providing support and education to individuals and families in the community who are struggling with chronic health conditions. I would also be responsible for connecting these individuals and families to resources and services within the community.
