To Whom It May Concern, I am writing in regards to the open position for Care Manager at Care Connect. Based on the job description, I believe that I am the ideal candidate for this role. I have experience in home care client case management, collaborative and person-centred care planning, new and existing client engagement, care plans, budget planning and reviews, care plan revision, assessments, person-centred goals and budget revision and management. My advanced IT skills, confident using computer and CRM systems, and ability to achieve KPI's and targets, will allow me to excel in this role. In addition, my understanding of Home Care Packages and Aged Care Standards, as well as my knowledge of services and networks for HCP and CHSP clients, makes me the perfect candidate for this position. What I love about this particular position is the focus on providing excellent customer service. I pride myself on my interpersonal communication and networking skills, and I am confident that I can provide comprehensive assessments and goals to Care Connect clients. I am available for a full-time role and my preferred locations are Wollongong and Shoalhaven.
