Provides spiritual support and counseling to hospice patients, their caregivers, and family members; assists patients and families in clarifying spiritual welfare and concerns; arranges additional spiritual support as needed and acts as liaison with patients' religious leaders or spiritual communities. Conducts initial pastoral care assessments within five days of admission, designs and oversees implementation of pastoral care plans, makes bereavement calls within two working days of death, provides bereavement support and assessment to bereavement coordinator, and conducts/supports hospice bereavement groups. Promotes hospice role and develops networks with clergy, churches, synagogues, and community organizations; participates as a member of the interdisciplinary team and attends scheduled meetings. Maintains professional documentation as part of the Plan of Care in the patient database system and performs other duties as assigned.
