Responds and investigates inquiries
complaints and emergency situations involving clients; assesses the situation and presents and implements short and long term solutions and documents outcomes.
Works with individuals and families and facilitates/mediates the resolution of difficult
complex issues the client is facing which are resulting in barriers to service access
effective service usage and/or autonomy (e.g. social
medical/psychological problems
tenancy
community problems
etc.); investigates and determines the cause
outlines implications regarding their choices and develops options/recommendations to reduce crisis situations and resolve issues
Advocates on behalf of clients for support
services and benefits with internal and external resources and engages in active case planning
case management and problem solving around each client’s barriers and difficulties
Provides support to individuals sheltering outdoors
including in encampments across York Region
including case management
working in partnership with external and internal partners
